You can modify Items on your academic record, such as your name, date of birth, foreign registration on number due to mistake or change by submitting Request for Modification of Academic Records' and a copy of documentary evidence to the administrative office of the under/graduate school you attend. The form 'Request for Modification of Academic Records' can be found in under/graduate school offices.
